**_care_** is seeking an experienced Care Liaison Officer in our Home Care Packages (HCP) team. Care Liaison Officers are responsible for supporting optimisation of the program through support to clients and program staff. This role works closely with the HCP team to enable provision of high-quality services to clients by providing day to day support to program staff, and coordination of service and support for clients. This is a 3-year contract with potential for extension.

Care Liaison Officers also undertake initial investigation of incidents from clients, reporting feedback, and actively participates in the implementation of processes that improve the client experience and program business processes.

**Essential**:

- Ability to assist and support program staff in the delivery of client care services.
- Experience in a related Community Services / Aged Care sector role.
- Excellent communication skills and ability to communicate with people of diverse cultural backgrounds.
- Strong organisational and administration skills.
- High level problem solving and investigation skills.
- Ability to prioritise tasks and follow through to completion.
- Initiative, attention to detail and accountability.
- Demonstrated ability to work independently or as part of a team, to achieve organisational objectives.
- Strong IT skills, including the Microsoft Office suite.
- Experience using Client database systems.

**Desirable**:

- Relevant tertiary qualification or equivalent experience
- Knowledge of Home Care Packages, CHSP, HACC guidelines and My Aged Care Portal.
