**_About the role_**

We are currently recruiting for a Disability/ Mental Health Support worker to work closely and provide care for our NDIS participants. Located in many area's across Melbourne. Our office is based in Carrum Down's VIC and work would be completed in various locations depending on your area of residence and the participants you work with. This Support Worker Role role directly contributes to the health and wellbeing of our clients through providing high quality care in home and community environments.

**_Duties will include: _**
- Assisting participants with daily living tasks and independent skills at home and in the community -including providing transport to appointments and activities.
- Provide personal care: personal hygiene and grooming
- Support with light domestic duties including meal preparations and cleaning
- Arranging social activities with participants
- Delivering individual support as per individual NDIS plans and goals
- Documenting services delivered, including but not limited to; shift notes, incident reports, medication management and more
- Experience in disability support and/or Mental health
- Current CPR & First Aid certification.
- National Police Check & NDIS Worker Screening Check
- NDIS Workers Orientation Certificate
- Valid Australian Drivers Licence & Own vehicle
- Excellent communication and interpersonal skills.
- Certificate in Disability Services/Community Services or relevant qualifications and/or experience
- Up to date COVID 19 Vaccinations
- Experience working with Autism preferred
