**The Opportunity**

Primary Care is transforming. Our Proactive Primary Care Project is a three-year investment and collaboration between **Amplar Health**,** Mediban**k and **Myhealth** - spearheaded by our amazing Myhealth General Practitioners This project is going to shake things up and transform the world of primary healthcare, especially for those with chronic conditions. GPs play a vital role in keeping us well and are the corner stone of an effective healthcare system, however, we know the sector is under pressure. This project is our way of innovating and supporting our GPs to deliver the best possible care.

Based in our Myhealth **Western Sydney clinics** you’ll work alongside our Head of Primary Care to lead the design and evaluation of new models of care with focus on:

- Informing the design and continuous improvement of new primary care interventions based on data driven insights
- Overseeing the collection, analysis, and presentation of clinical and non-clinical data for evaluation by third parties including university researchers
- Enabling accurate measurement of the effectiveness and impact of new interventions, communicating findings to internal and external stakeholders effectively including government departments and advocacy groups
- Driving the identification of trends and insights in data to steer positive project outcomes efficiently

This is a 2-year fixed term contract based in Western Sydney with a salary range of $110-130k.

**About You**

This role suits someone with a mix of healthcare knowledge (preferably models of care) and some analytical capability. You may have been working in Consulting around healthcare transformation/strategy and/or working within LHD/PHN’s providing exposure to health data.

Ideally your resume will showcase your project management and stakeholder engagement skills - which are critical for this project.
