We have an exciting opportunity for experienced and motivated Personal Care Assistants to join our Home Care Services team on a casual basis. You will be working within the community setting and responsible for the delivery of personal care and assistance with activities of daily living to community based clients in accordance with their personalised care plans, supporting them to live independently and optimise their health and well-being.

**Key Responsibilities**:

- Deliver planned individualised care to clients in accordance with the client’s care plan. Care includes but is not limited to; shower/bathing assistance, toileting, dressing and grooming, skin care, oral hygiene, shopping, mobility, housework, preparation of meals, providing community access, companionship, assistance with self-administration of medication and fitting of aids/appliances such as splints and hearing aids.
- Respect the client’s dignity and choice at all times, observing any cultural preferences or diversity.
- Be familiar with allocated client’s care plan prior to attending the visit and ensure the care plan is delivered to meet their chosen goal.
- Respect each client’s privacy and keep all personal information confidential.
- Report any concerns and changes in the client’s behaviour, living and health condition to the allocated Case Manager immediately.
- Consistently demonstrate behaviours that reinforce St Luke’s Care Values of _**Excellence, Transparency, Integrity, Respect and Trust**_, inside and outside of the workplace.

**Essential Criteria**:

- Have a functional smart mobile phone at all times
- Maintain an unrestricted driver’s licence, comprehensive car insurance and car registration
- A commitment to excellence in service/care delivery
- Demonstrated professional/positive behaviour, including cultural sensitivity and respect
- Ability to work both autonomously and as part of a team
- Effective verbal, non-verbal and written communication skills

**Desirable Criteria**:

- Certificate III or IV in Individual Support (Ageing)

**Job Types**: Part-time, Casual
Part-time hours: 30 per week

**Salary**: $25.00 - $35.00 per hour
