Case manager -AUCARE ROCHEDALE SOUTH OFFICE

AUCARE is seeking a Case Manager to join our community NDIS and aged care team, on a permanent part time or full-time basis. DAY TIME OFFICE WORK, NO WEEKEND SHIFT

As a **Case Manager** provide person-centred care to your clients in the community. The main duties include:

- Assess, plan and implement client care within your scope
- Consider physical, social and psychological requirements
- Create and update care plans to reflect developing needs
- Ensure clinical quality compliance standards are met
- Assist with rostering and HR

**What Do You Need?**
- Excellent communication and clinical problem-solving skills
- Confidence in managing your time effectively, including an ability to work autonomously.
- Blue Card, police check and NDIS Workers Screening Clearance or willingness to obtain
- Unlimited work entitlements
- Current Australian Driver’s Licence and own car

**Job Types**: Full-time, Part-time

**Salary**: $30.00 - $36.00 per hour

Schedule:

- Monday to Friday
- No weekends
