**Our Opportunity to you**

The Community Connect Specialist plays a crucial role in effectively deploying clinical workers to clients in the community.

This **hybrid** role will be working out of our **Brisbane Office **and from home on a **permanent part time basis.**

**The day to day**
- Responsible for successful onboarding of new clients, document management, client liaison, effective scheduling of allied health and nursing services, and ongoing client case support throughout your client’s Remedy journey
- Credentialing and management of new service providers, liaising with and arranging providers to deliver care to our clients in their home
- Providing quality administrative support to your team, including reporting, document processing and client data management

**About you**
- Self-motivated medical and office administrator, skilled in customer service
- Strong administration and time management skills, comfortable with using multiple systems
- Experience in scheduling client and provider services, along with managing inquiries effectively
- Effective communication skills and building rapport with external stakeholders
- Adaptable and solution focused with the tenacity to overcome challenges
