**About the role**

As the Placement Services Welfare Officer, you will play a critical role in helping patients and their families navigate the complexities of aged care placement and support their safe return to community living. Reporting to the TCP Service Manager, your key responsibilities will include assisting patients with accessing the benefits and services needed to support their hospitalisation, discharge, and ongoing care. You will work closely with resource providers, negotiating and coordinating the necessary services to ensure a smooth transition for each patient.

In addition, you will be the primary contact for managing relationships with aged care facilities, maintaining up-to-date knowledge of available options, and liaising with both facilities and families. You will also provide vital support to clinical and multidisciplinary teams in the placement of patients into appropriate residential or community care facilities. **Key Requirements**:

- **Independent & Initiative-Driven**: Able to work autonomously, use your initiative, and advocate for patients with a range of organisations and departments.
- **Effective Communication & Interpersonal Skills**: Strong communication skills, with the ability to relate to clients from diverse backgrounds and support those in distress.
- **Experience in Aged Care & Hospital Settings**: Previous experience working with patients and families in aged care or hospital environments, particularly in placement coordination.
- **Organisational & Time Management Skills**: Excellent at prioritising tasks, planning, and negotiating to deliver the best possible outcomes for patients and their families.
