Our Lifestyle Club Support Workers support clients to have fun interacting with others in a comfortable home-like environment. Prepare and facilitate events to increase socialization, relaxation and recreation among the clients that visit.

**The role is responsible for**:

- Providing transport to and from the center in vehicles provided by HALO Care & Support.
- Assist with planning and facilitating daily, weekly and monthly events to encourage clients in socialization, relaxation and recreation.
- Assist all individuals to maintain their current capacity and/or re-enable skills through structured activity and stimulation.
- Encourage the individual to participate in their support so that they maintain as much independence as possible.
- Assist with assessments, review and the ongoing evaluation of clients cultural, recreational and leisure needs and lifestyle interests.
- Participate in the development and delivery of lifestyle programs to our clients, ensuring residents are able to participate and enjoy the programs.
- Implementing activities on a daily basis ensuring participation by clients, and providing relevant feedback
- Assisting clients with activities of daily living as required
- Completion of documentation as required

**We are looking for someone who has the following**:

- Drive and enthusiasm to provide outstanding supports in a center based setting
- Bubbly, approachable personality who thrives in a team environment
- Certificate III in Aged Care and at least two years' experience in a similar center based setting.
- Current First Aid and CPR certificate (or willing to obtain one)
- Current Medication Competency certificate
- Working With Children Check
- A valid full (non-provisional) driving licence
- NDIS Worker Screening Check
