This amazing opportunity will see you providing health coaching and support to HCF members in relation to HCF programs in order to improve member health and wellbeing and minimise hospitalisation.

This involves providing telephone and video based support to connect members with HCF resources, helping them navigate the health care system, providing evidence based advice on health and wellbeing behaviours and deliver relevant disease management programs such as the COACH cardiovascular chronic disease management program to appropriate members.

**Key responsibilities**
- Responsible for recruitment of appropriate members onto programs where appropriate and follow up calls.
- Provide coaching over the phone and video to HCF members in relation to nutrition and diet and many other health and wellbeing programs
- Engage members on the COACH and other programs through the phone and video
- Provide a consultation service to other departments such as Front Line, Ancillary Benefits, Management and the Product team.

**About you**

To be successful in this role, you will demonstrate the following qualifications, experience and skills:

- Highly experienced Registered Nurse, Pharmacist or Dietician with recent experience and or Corporate experience that has a good understanding of the health care system in Australia.
- Experience working in specialised health sector fields (desirable)Good interpersonal skills and an engaging manner on the telephone
- Skilled at conflict management and resolution
- Skilled at encouraging behaviour change in others
- Ability to assimilate new data and to assess the veracity of medical information and reports
- Previous experience working for a company that delivers care telephonically is desirable
