- Building positive relationships with clients, stakeholders, and team members
- Supporting clients with their individual needs and goals
- Developing care plans and managing client case loads
- Ensuring delivery of effective support and alignment with company values and policies
- Responding to client feedback and managing client issues and outcomes Requirements
- work closely with clients on a range of package levels and use their case management skills to conduct client-centered assessments

**Skills and Experience Requirements**:

- A relevant qualification and/or 2+ years of experience in case management, allied health, nursing, or social work
- OR a background in logistics, banking, and the corporate sector, with a background in managing accounts and clients
- Current driver's license and police check
- Strong customer service focus and confident, creative approach
- Strong budget management skills and problem solving ability
- Intermediate computer skills and experience with client management systems
