**About the Role**

Assist NDIS participants in the support coordination and implementation of supports across their funded plan to strengthen and enhance the client’s abilities and goals. This role has full flexibility to work from home and will be required to travel across Melbourne to successfully deliver the clients plan and meet their obligations.

**Key Responsibilities**
- Support our participants to implement their NDIS plan
- Monitor plan outcomes and expenditure to ensure it remains within budget
- Ensure mainstream services meet the participants NDIS plan
- Build and maintain positive relationships with participants, support workers and external stakeholders
- Report outcomes and success indicators to the NDIA within the agreed reporting timeframe
- Strengthen and enhance the participants abilities to self-direct their own supports and participate in the community
- Attending disability expos, forums, open days and networking events to proactively promote Windsor Health and what we do
- Drive the development of new business by implementing strategies to consolidate partnerships and referral streams and contribute to participant retention
- Contribute towards securing new participants along with connecting them with appropriate support workers based on participant requirements.
- Build relationships with new referrers/partners/clients which drive business growth

**Skills & Experience**
- Minimum 3 years experience as NDIS Support Coordinator
- Working knowledge of the National Disability Insurance Scheme objectives and principles
- Excellent communication skills and the ability to engage with support workers, clients and their families
- Passion to make enhance people with disabilities lives for the better
- Experience working autonomously
- SIL knowledge preferred
- Must have an unrestricted Victorian drivers’ licence and reliable car

Pay: $45.00 - $50.00 per hour

Expected hours: 6 - 20 per week
