Transition Care Program/Restorative Care Case Manager - Casual Position
- Repatriation Campus, based onsite

An opportunity currently exists for an exerienced and passionate nursing or allied health professional to join our Community Aged Care Services team in a casual capacity
**About the role**
- Working within the Community Aged Care Services Team, supported by the TCP Team Leader, with peer support from other case managers leader the role would see you:
- Completing detailed individual assessment and care planning for eligible older clients of the Austin Health Transition Care or Restorative Care Programs focusing on the client’s abilities, lifestyles and interests
- Providing a period of case management, intensive service provision and / or therapy within their own home or aged care facility, enabling discharge to other community supports or to an aged care facility at optimal level of functioning
