**About the Role**
Reporting to the Medicine Service Manager, the CNC/CNS is responsible for contributing to quality health care by collaborating with the multidisciplinary health care team and consumer groups, developing clinical practice standards and evaluating their outcomes. The stroke CNC/CNS is an expert practitioner who, through leadership, facilitates research based clinical practice, which is creative, visionary and relevant to our in-patient service. More specifically your responsibilities will include:

- Provide leadership in planning, development, implementation and evaluation of stroke pathways and care
- Participation in strategic and operational planning of stroke services across NBH
- Provide education and support to the TIA clinic
- Promote stroke nursing, fostering excellence through clinical, educational, research and professional relationships.
- An expert in stroke management contributing to quality healthcare through collaborative stroke management
- Collaborating with the Emergency Department to ensure timely access to stroke management of all stroke presentations.
- Formulate and implement policy, guides practice development and engages in education and research activities.
- Provides hospital wide consultancy, promotes evidence based care within a collaborative multidisciplinary team
- Provides advanced clinical support to develop, facilitate, implement and evaluate care plans to meet the needs of the patient related to the management of stroke admissions

**To be successful**:

- Registered with the Australian Health Practitioner Regulation Agency (AHPRA)
- Bachelor of Science (Nursing) or equivalent
- Post graduate qualification in the relevant specialty field at degree level or higher (or equivalent)
- At least 5 year’s full time post registration experience & at least 3 years’ experience in the neurology and stroke field
- Previous experience as a CNC/CNS or acting in similar roles
