**About the Role**

We are seeking **compassionate, professional, and reliable Community Support Workers** to provide direct care and support services to individuals in their homes and local communities. Your role will be essential in supporting clients to achieve their goals, maintain independence, and participate fully in life.

**Key Responsibilities**

Deliver person-centred in-home and community-based support
Assist with personal care, domestic tasks, meal preparation, and medication prompts
Support social participation, transport, and recreational activities
Assist clients with mobility, transfers, and safe use of equipment
Maintain accurate progress notes and communicate changes in client needs
Promote dignity, safety, independence, and inclusion at all times
Follow care plans, risk assessments, and Your Care Co policies
Engage respectfully with clients, families, and professionals

**What We’re Looking For**

✔ Genuine compassion, empathy, and commitment to client rights
✔ Ability to work independently and professionally in diverse environments
✔ Excellent communication and documentation skills
✔ Availability for a variety of shifts, including weekends and sleepovers

**Please note**: Reliable transport is required to travel between client homes.

**Qualifications & Experience**:

- Certificate III in Aged Care, Individual Support, or similar
- OR Certificate IV in Mental Health, Disability, Community Services
- OR currently in 2nd year of Nursing or Allied Health at university
- Current flu vaccination (evidence via Medicare)
- Level 2 First Aid & CPR (current)
- National Police Check (within last 3 months)
- Working with Children Check
- NDIS Worker Screening Check
- Australian Driver’s Licence, roadworthy vehicle, and comprehensive car insurance
- Own a smartphone
