**About the Role**:The Home Support Service Planner works collaboratively with customers by co-designing a goal-orientated support plan, based on the recommended services set by the Regional Assessment Service (MAC). The role provides information and coordination of services to support customers to remain living at home with independence.

**What you'll do**:

- Co-design services with customers based on their goals, reflecting their individual needs
- Identifying and responding to risks and enabling customers to stay healthy and independent whilst remaining at home
- Conduct reviews, develop support plans, refer on for identified clinical and care intervention needs
- Meet individual KPI's and contribute to team targets
- Act as first contact for resolution for customer concerns and complaints
- Liaise effectively and positively with external stakeholders such as RAS, MAC and ACAT

**You will need**
- Certificate III in Community Services/Aged Care/Service Coordination or relevant Health Professional Qualification or equivalent experience
- Knowledge of issues associated with ageing, including dementia related disorders
- Intermediate/advanced level knowledge of Microsoft Office
- Excellent documentation and administration skills
- Enthusiastic team member and problem solver
