Glynde, Australia LHI Retirement Services Full time

The Commonwealth Home Support Program (CHSP) Coordinator works closely with the Home Care Manager, Home Care Quality and Education Coordinator and Leading Therapies Teams, to ensure the coordination and optimum customer service of LHI Retirement Services (LHI) CHSP program.

The CHSP provides entry-level support to assist older people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander peoples) to remain living at home and in their community.

The CHSP funds
- domestic assistance,
- transport,
- meals,
- personal care,
- home maintenance, social support,
- nursing and allied health.

The CHSP Coordinator coordinates the CHSP relationships through planned respite services for older people, placing a strong focus on activities that support independence and social connectedness and takes into account each person’s individual goals and choices.
