**About the Opportunity**

We seek an experienced, compassionate Clinical Care Manager to coordinate care services for elderly clients at ECH. You’ll manage a specialised caseload with complex clinical needs. You will ensure clients are informed and supported by navigating services and funding models, enabling clients to live confidently and comfortably at home. Responsibilities include conducting comprehensive assessments, creating tailored plans aligned with wellness goals, and managing client relationships and feedback, escalating issues to the management team where necessary.

**About You**

You will have a Qualification in Allied Health or Nursing and recent clinical experience working in a related area

You have a deep commitment in providing compassionate care to our elder clients.

You are sensitive to the needs of older people of different social, economic and culture backgrounds.

You are able to identify when a client’s needs have changed to ensure they continue to receive appropriate care.

You are kind, even when a bit stressed out, dynamic and energetic. You are also flexible and adaptable to change.

You own your work and take responsibility for your own actions, learn from mistakes, and proactively seek opportunities to contribute, innovate and solve problems.

You are passionate about caring for people in the community and will contribute your ideas and hard work to make ECH better every day.
