**About Chorus**

Chorus is a WA-based organization with close to 1000 team members and volunteers who support seniors, people on the mental health recovery journey, and those living with disabilities. Our purpose is to enable people to live the life they choose at home and in the community. We’ve designed a structure that focuses less on management and hierarchy and more on the things that matter - relationships, customers, and communities. Our network of small local teams enables Chorus people to be more connected to each other and feel a sense of belonging.

**About the role**

**Essential Requirements**:

- Minimum Certificate III in Disability, Individual Support;
- NDIS Module or willing to obtain;
- At least 12 months of recent experience working in a disability community setting and experience in an Accommodation/Supported Independent Living setting is also desirable;
- Experienced in supporting people with complex needs; including personal care and all aspects of daily life;
- Have experience in implementing Positive Behaviour Support Plans;
- Advocate for people living with a disability;
- Enthusiastic and engaging with customers;
- Proven ability to provide support to a number of customers in a group setting;
- Excellent communication skills both written and verbal to interact with a diverse range of customers, their families, carers, community agencies, and other professionals;
- Understanding of professional boundaries and privacy and confidentiality issues;
- Ability to work as part of a team;
- Good computer & mobile device skills;
- A proactive approach to the safety and well being of yourself and customers;
- Current Australian Drivers Licence with no restrictions;
- Current First Aid Certificate (12 months or less currency) or willingness to apply;
- A roadworthy vehicle in order to transport customers;
- Working in Supporting independent living
- Support Workers are required for varying shifts across weekdays and weekends with sleepover shifts also available. This is a 24/7 independent living setting and flexibility is essential;
