**Roles and Responsibilities**:
**Client Assessment and Support**:

- Assess individual client needs and collaboratively develop tailored care plans that align with their goals and support requirements.
- Facilitate educational, training, and support programs to enhance independence and well-being.
- Interview clients and their families to identify specific challenges and offer solutions.
- Provide ongoing monitoring and reporting on client progress, adjusting care plans as needed.

**Community Engagement**:

- Liaise with community groups, government agencies, and private businesses to raise awareness about available resources and services.
- Promote inclusion by organizing and supporting community-based programs for clients to participate in meaningful activities.

**Resource Coordination and Advocacy**:

- Assist clients and families in accessing services related to health, welfare, housing, employment, and training.
- Advocate for clients by preparing funding submissions and resource requests, ensuring they receive appropriate assistance.
- Refer clients to specialized agencies or professionals when additional support is required.

**Crisis Support and Case Management**:

- Provide emotional and social support to individuals and families facing crises.
- Supervise and support individuals in government institutions, group housing, or adult service units.
- Assist young people and families in addressing social, financial, and emotional challenges.

**Health and Safety**:

- Ensure compliance with workplace health and safety guidelines for all activities.
- Identify risks and report any incidents promptly, ensuring the safety of clients, staff, and community members.

**Requirements**:

- Valid work visa holders with full work rights
- Driver’s license and access to a vehicle
- National Police Clearance Certificate (not older than 3 months)
- NDIS Worker Screening Check
- Covid-19 Vaccination/Booster/Flu Vaccine
- Working with Children Check
- CPR and First Aid Certificate (not older than 12 months)
- Experience in community development, welfare services, or related fields is highly regarded.

**Job Types**: Full-time, Permanent

Pay: $75,000.00 - $85,000.00 per year
