**Support and Outreach Coordinator**

**Role**: Support and Outreach Coordinator - Diggers Rest, VIC
**Hours**: 8-10 hours per week (Part-Time)
**Must be available Mondays and Fridays**

**Key Responsibilities**:
**Support Responsibilities**:

- Provide individualized support to Participant, assisting with daily living activities such as personal care, meal preparation, and household tasks.
- Assist Participant with emotional regulation, especially during heightened periods of anxiety, ensuring his physical and emotional well-being.
- Accompany Participant to community activities, including outdoor recreational activities like fishing, while ensuring his participation is safe and engaging.
- Collaborate closely with Participant family and our team to implement structured, person-centred support strategies, focusing on managing Participants intellectual disability, Cornelia de Lange Syndrome, severe anxiety, and verbal dyspraxia.
- Assist Participant in transition back to part-time employment, helping him manage his schedule while supporting his emotional health.
- Work alongside a male support worker to provide a balanced care approach, aligning with Participants preferences.

**Outreach Responsibilities**:

- Engage in localized outreach to promote NDC services and increase community awareness of the support offered for participants like James.
- Utilize social media and other platforms to highlight support services, upcoming events, and success stories to engage the community.
- Combine support work with outreach activities, contributing to the growth of NDC’s presence and service awareness in the local community.
- Collaborate with a dynamic, innovative team to ensure the highest quality support services.

**Requirements**:

- Outgoing, positive, and resourceful attitude.
- Compassionate and patient, with experience working with individuals with intellectual disabilities, severe anxiety, and behaviors of concern.
- Self-motivated with the ability to work both independently and as part of a team.
- Relevant experience as an NDIS Support Worker, with a strong understanding of person-centred support.
- Ability to match client preferences for gender (both male and female support workers needed) and personality traits (positive, calm, motivational).

**Skills and Qualifications**:

- Must meet Participant preference for both a female and male support worker.
- 100 Points of Identification.
- Valid Driver's License with access to a reliable vehicle.
- Car Insurance and Registration.
- NDIS Workers Screening Clearance.
- NDIS Workers Orientation Module Certificate.
- Police Check.
- Working With Children Check (WWCC).
- First Aid and CPR Certification.
- Experience in the disability sector, with a focus on intellectual disabilities, anxiety management, and emotional regulation.

**How to Apply**
