**The role**

Reporting to the National Transitions & Clinical Manager, the Transition and Clinical Specialist will work to conduct an initial assessment of participant/s to check the viability of the organisation's capacity to deliver service.

In particular, you will be responsible for the following:

- Supervise and monitor the maintenance of clinical & medical records and other care-related documentation for all QLD participants
- Review support plans and health assessments facilitating change as necessary to meet client needs
- Lodge, monitor and maintain submissions for all NDIS-related restrictive practice authorisations, and carry out monthly reporting of restrictive practice usage
- Prepare and develop transition plans for future intakes

**About you**

As the Transition and Clinical Specialist you are a strong problem solver as you have the ability to use your critical thinking skills to make decisions. You also have exceptional attention to detail and take initiative to work through cases and incidents.

- Demonstrated experience within a transition and clinical specialist role, or similar
- Excellent communication skills, both written and verbal
- Ability to work under pressure managing multiple priorities
