The Department of Justice and Community Safety is seeking **4 x Assessment and Transition Co-ordinator's** to provide a critical role at Western Plains Correctional Centre.

**About the Business Unit**

Western Plains Correctional Centre is a modern, maximum-security prison focussed on rehabilitation, housing up to 1248 people in custody. Western Plains features multi-purpose infrastructure which is adaptable to the evolving needs of Corrections Victoria and the people in our care. **About the Role**

The Assessment and Transition Co-ordinator (ATC) forms part of the multi-disciplinary Offender Services team, which is responsible for the development, co-ordination and delivery of a range of programs and services across several specialist portfolios: transition and reintegration, family engagement, disability, diversity and complex care.

Some of your duties will include:

- Support the delivery of the Corrections Victoria Reintegration Pathway in a prison environment.
- Administer prisoner risk and needs assessment tools to effectively identify rehabilitation and reintegration requirements, making referrals to relevant programs and services as required.
- Work collaboratively with multi-disciplinary teams to develop integrated and effective transition and other specialist support plans, including actively involving and motivating prisoners to engage in goal setting and program/service participation.

**About you**

To be successful in this role, you will have:

- Demonstrated understanding of the specific transition and reintegration issues and needs of prisoners.
- Demonstrated ability to administer risk and/or welfare assessment tools.
- Effective stakeholder management skills.
- Ability to communicate sensitively and effectively with diverse cohorts, especially Aboriginal and Torres Strait Islander communities.
- Demonstrated capability to work with people who are difficult to engage individually, and in group settings.

**Qualifications/requirements**
- A tertiary qualification in an appropriate discipline is highly desirable.
- Demonstrated skills and experience in human services, welfare practice or related fields are highly regarded.
