We strive to empower our participants to live life as independently as possible and offer qualified support staff to work with our participants, their families and carers.
We are expanding our Support Coordination team. You will be working from our comfortable Canberra Offices with options for flexible working conditions from home, as well as travelling to see clients in their homes when needed. Majority of our client are in the Canberra region, NSW and Victoria.
**About the Role**
**You will need**:
- Experience in previous support coordination or case management role
- Minimum Certificate IV in Mental Health, Disability or similar, currently studying or willingness to achieve same, or higher relevant qualification preffered
- Minimum 3 years industry \experience.
- Full current Driver licence.
- Rights to work in Australia
- A current valid Driver's License
- Satisfactory Police Records Check [less than 12 months old] and
- Working with Vulnerable People Check, prior to commencement is mandatory
- NDIS Workers Screening Check and NDIS Worker Orientation Module Certificate is mandatory prior to commencement
- Strong computer skills, with experience in Microsoft Office, video conferencing and client data management systems.
- Demonstrated ability to build and sustain relationships with all relevant stakeholders.
- Excellent written and verbal communication skills, including the ability to write detailed reports.
- Knowledge of and ability to research relevant service providers and services in the areas.
- Ability to work independently and be able to prioritise
- A very good understanding of how NDIS works and remain up to date with changes. An understanding of the Support coordination role including the roles parameters and limitations.
- Open, honest and respectful communication with a mature outlook and ability to give and receive feedback for constructive personal development.
- An understanding of billable hours and KPI’s.
- To be able to demonstrate sound understanding of professional boundaries.
**Duties and Responsibilities.**
Your day-to-day tasks include, but are not limited the following duties:
- Support implementation of all services in a participant’s plan, including informal, mainstream and community, as well as funded supports.
- The coordination and collaboration between multiple stakeholders from formal, mainstream, community, and informal supports. Chairing meetings for all stakeholders as needed.
- Support participants to understand all aspects of their NDIS plan, as well as assist participants to navigate the NDIS provider network, sourcing & implementing the required services and supports to facilitate progression towards achieving their goals.
- Networking and liaising with stakeholders across a number of platforms and conducting yourself professionally at all times as a representative of the company, encouraging repeat business.
- Research, coordinate and manage complex supports for the participant across multiple providers, ensuring client choice and control is always upheld when making referrals.
- Carry out thorough risk assessments for each referral.
- Identify and seek relevant assessments and associated service design.
- Capacity building of clients to ensure they achieve their goals as per their NDIS Plan
- Identify and resolve barriers to service provision, along with managing times of crisis.
- Ensure mainstream services meet their obligations (i.e., housing, education, justice, health)
- Provide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency.
- Maintain professional case notes, documentation and report writing.
- Support the participant to ensure they get the best plan possible for their needs
**Job Type**: permanent full/part time
**Salary**: $75000 per annum

**Salary**: $65,000.00 - $75,000.00 per year
Expected hours: No more than 40 per week
