**The Opportunity**

As our Clinical Case Manager, you will lead the coordination of tailored, safe, and effective care plans. You’ll be the key point of clinical support for clients, families, and staff, ensuring care delivery aligns with the Aged Care Quality Standards, NDIS frameworks, and best practice principles.

**Key Responsibilities**
- Conduct comprehensive client assessments and risk reviews in home settings
- Develop, review, and manage personalised care plans in line with funding (HCP, STRC, NDIS)
- Provide clinical oversight and mentorship to care staff, ensuring delivery of safe and responsive services
- Actively lead case conferences, care reviews, and service plan updates
- Liaise with GPs, allied health, hospitals, and families to coordinate multidisciplinary input
- Deliver nursing care in the community, including wound management, continence care and clinical monitoring
- Monitor documentation for accuracy, quality compliance, and to support optimal funding outcomes
- Support with onboarding, supervision, and ongoing professional guidance for field workers and junior nurses
- Play an active role in clinical governance, quality improvement, and audit preparation
- Maintain accurate electronic records using our clinical system.

**What We’re Looking For**
- Registered Nurse with AHPRA registration (unrestricted)
- 2+ years of experience in case management for Home Care Packages (Bonus if you also have experience managing Short-Term Restorative Care)
- Proven understanding of Aged Care legislations, NDIS Quality & Safeguarding Framework, and care planning processes
- Strong interpersonal skills and the ability to support staff, clients, and families with empathy and professionalism
- Competence with electronic documentation systems and intermediate computer skills
- Valid Australian driver’s licence and reliable transport
- Ability to lead and contribute to a positive, respectful team culture
- National Police Check and NDIS Worker Screening (or willingness to obtain)

Pay: $88,000.00 - $105,000.00 per year
