**Position Overview**:
Within the framework of the KNC’s philosophy and objectives, community staff will work with the Centre Manager and coordinators to co-ordinate the delivery of Community Support to people in the Northern Sydney region, and carers of such people, who because of their frailty, age or disabilities are at risk of moving prematurely into residential care.

**Duties**:

- Work collaboratively with other staff to ensure the KNC and Community Service runs effectively and efficiently on a day-to-day basis
- Liaise with clients and volunteers regarding the activities of Community services, ensuring they are in accordance with the philosophy, objectives and policies of KNC and MAC.
- Ensure duty of care is met for all clients and volunteers
- With other staff coordinate the Community Service Program according to Department of Health / CHSP/ DSS Commonwealth Home Support Program (CHSP) guidelines ensuring that
- Community Care client CIARRs, support plans and reviews as per KPI, records and data are maintained and regular reassessment of clients occur
- Appropriate response to enquiries and referrals to Commonwealth Home Support Program and referral of people and clients to other services as required

**Staff and Volunteer Management**:

- Roster volunteers for specific drives in Community Services
- Provide adequate and appropriate information, supervision and support to all volunteers and clients
- Ensure the provision and maintenance of safe working conditions for all staff, clients and volunteers
- Plan Wellness activities - plan centre-based and social group activities to ensure they are customer focused, responsive and promote wellness and re-ablement concepts where appropriate
- Manage Wellness service delivery - coordinate and supervise Wellness services including, recordkeeping, quality, risk management and WH&S
- Maintain knowledge of, and work cooperatively with, all KNC service delivery areas
- Demonstrate leadership and promote KNC services to the wider community
- Maintain Consumer Rights, Privacy and Confidentiality
- Demonstrated computer literacy, Microsoft Office Suite
- Demonstrated ability to work with volunteers
- Ability to work as part of a team
- Ability to work with a management committee
- Excellent written and verbal communication skills

**Job Types**: Full-time, Permanent
