**About the position**

Provision of case management, assessment, and ongoing support to minimise the harms of alcohol and drug use to young people from a CALD background primarily South Sudanese, African and Pasifika background residing in the Western Metropolitan Region.

**Some of your key responsibilities will be**
- The provision of developmentally appropriate and culturally sensitive AOD support to CALD young people within the Western Metropolitan region
- Initiate and maintain positive relationships with a broad range of stakeholders and develop options for implementing a partnership approach to the provision of service delivery
- Plan, develop and establish care pathways for young people from CALD backgrounds residing in the Western metro region
- Manage a caseload of up to eight clients
- Partake in community development initiatives
- Complete comprehensive assessments and care plans
