**Role Overview**:
We are currently seeking a dedicated and proactive Team Leader / Support Worker to oversee the day-to-day operations of a Supported Independent Living (SIL) home. This hands-on leadership role involves supporting participants with complex needs while supervising a small team of support staff to ensure high-quality service delivery, compliance, and consistency.

**Key Responsibilities**:
Oversee the daily operations of the SIL home, ensuring a safe, person-centred, and supportive environment.

Provide direct support to participants with personal care, daily living activities, and community access, as required.

Lead and support a team of support workers, including shift planning, mentoring, performance feedback, and communication.

Ensure all supports are delivered in line with participants’ individualised plans, goals, and NDIS practice standards.

Monitor and document participants' wellbeing, progress, and any incidents or changes in behaviour or health.

Manage staff rosters, shift handovers, and coordinate with the broader management and clinical teams.

Assist with medication administration, transportation, and household tasks within the SIL.

Providing personal and community care. i.e. Medication administration, manual handling, meal preparation, transportation.

Promote independence, dignity, and choice for all residents.

**What We’re Looking For**:
Experience in a leadership or senior support role within disability or aged care preferred but not mandatory

Strong understanding of NDIS and SIL compliance standards.

Exceptional communication and organisational skills.

Ability to work autonomously and manage competing priorities.

Empathy, reliability, and a passion for person-centred care.

**Key Selection Criteria**:

- **Essentials**:_
- Extensive experience in the provision of supports for people with a disability and mental health.
- Highly developed communication and inter-personal communication skills including the ability to work and communicate effectively with the people we support, families, communities, and other professionals.
- NDIS Workers Screening Check.
- Valid Victorian Driver’s Licence.
- First Aid and CPR certification.
- Working with Children’s Check.
- Tertiary Qualifications relevant to the disability/mental health field.
- Minimum of 2 years qualification and working within the field.
- Experience in the disability/mental health sector.
- Manual Handling & Hoist experience.
- Excellent verbal and written communication skills in English

Pay: $33.00 - $43.00 per hour
