ABOUT THE ROLE
We are seeking an experienced Disability Support Coordinator to join our team at Port Phillip Prison, to play a pivotal role in creating a positive and supportive environment for people with a cognitive impairment within the prison. In this role. you will ensure programs and services focus on the individual needs, interests, and aspirations of offenders with a cognitive impairment. You will work closely alongside the broader justice and disability service system and other mainstream services, to address disability specific factors contributing to a person's risk of offending, and utilise this information to best develop our rehabilitation framework.

**Daily duties will include**:

- Develop, coordinate, implement and monitor programs and services specifically designed for prisoners in our care
- Build strong relationships and collaborate with internal and external stakeholders, including service providers and community organisations
- Prioritise the principles of the Good Lives Model (GLM) to ensure programs and services focus on individual needs, interests, and aspirations
- Advocate for a person-centered approach to rehabilitation, assisting offenders in developing skills and resources to reduce their risk of re-offending

ABOUT YOU
- Certificate III or above in Disability/Individual Support
- Sufficient experience in coordinating and implementing disability programs
- Experience working with people with: physical, sensory, psychosocial or intellectual impairments
- Knowledge of prioritising human rights and positive behaviour support in program coordination
- Understanding of offending behavior and the specific needs of offenders, is desirable
