**About the Role**

You will provide personalised support to our QCSS (Queensland Community Support Scheme) clients within their homes in Toowoomba and surrounding local areas. You will be required to travel to support multiple clients each day within Toowoomba and surrounding local areas such as Gatton and Oakey (travel between clients will be reimbursed with.99 cents per kilometre travelled while you work back in your pocket).

You will provide support to clients that have complex medical needs and need mental health support. This is a Permanent Part Time opportunity with working hours available between 7am to 5pm, Monday to Friday, no evening or sleepover shifts. There is some flexibility for hours to be within school hours.

**Key Responsibilities**

Assist clients to improve their independence according to their identified needs and goals inclusive of:
In home supports:

- Domestic assistance such as light cleaning and housework
- Personal care
- Assistance with meal preparation
- Skill development

Community Connections supports:

- Social support & linking
- Shopping assistance
- Assist clients to improve their independence in according to their identified needs and goals
- Establish a rapport and collaborate effectively with clients and their family

**Skills & Experience**
- Certificate III in Individual Support (Ageing, Disability or similar)
- QLD Working with Children Check (Blue Card)
- Excellent organisational skills, hygiene practices and problem-solving abilities
- An Australian Driver’s Licence and reliable vehicle with minimum third-party insurance
- Strong communication skills with the ability to build and maintain positive relationships
- A current First Aid and CPR Certificate (must be obtained prior to employment)
