**Description**:
About the role

The NDIS Support Coordinator’s key role function is to coordinate support services for NDIS clients seeking assistance from ATSICHS. The role will assist in engaging support services while building capacity and enabling participants to live more independently and be included in their communities.

Duties

All NDIS participant details and information around NDIS plans and case notes are well documented in Brevity.
Maintain accurate and detailed case notes of all engagement with clients and other stakeholders.
Working with Participants to achieve their goals in line with their NDIS plan.
Develop, implement, monitor and review ATSICHS NDIS Service Level Agreements (SLAs) and where applicable, ensure participants receive timely and clear communication for SLA’s and quotes for participants.
Create linkages with the community through communication and marketing and community engagement.
Maintain systems and undertake process enhancements.
Ensuring compliance with relevant legislation, guidelines and ATSICHS policies procedures and values.
Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships, facilitates cooperation, trust and respect, values differences and diversity and guides, mentors and focuses on the development of stafff.

- Permanent full time position with annual salary of $75,531.
- 10.5 % Super + Not for Profit Salary Packaging (Increases your take home pay).

COVID-19 Requirements
- Immunisation history showing you have received two doses of a Covid -19 vaccinine.

**Skills and Experiences**:
Required

Certificate lV or higher qualifications in Health Care, Social Assistance, Community Services or Administration.
Knowledge, understanding and/or awareness of the NDIS space and the challenges the participants may face.
Demonstrated ability to record accurate and consistent case notes.
Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
Strong organisational skills and ability to work independently and/or as part of a team.
Previous experience in working with culturally diverse people.
Ability to communicate sensitivity and effectively with Aboriginal and Torres Strait Islander people.
Sound knowledge of Microsoft Suite Applications.
Current Drivers Licence (Qld).
Current National Police Clearance or ability to obtain.
Working with Children’s Card (Blue Card) or be eligible to apply.
Yellow card or be eligible to apply.
