We are currently seeking a values-driven and dedicated Care and Recovery Coordination and Mental Health Case Manager. This position is a dual-purpose role, responsible for providing care and recovery coordination (CRC) through a range of interventions including case management to assist in improving the health and wellbeing for individuals, family members and significant others experiencing alcohol and other drug related harms referred to Barwon AOD & Mental Health Services within The Salvation Army.

In addition, this role will also provide support and case management to clients in the Accommodation & Psychosupport Program (APPS) who are the accessing transitional accommodation following exit from Barwon Health acute mental health services and provide mental health psychosocial secondary consultation to TSA Services in the Barwon region.

You will successfully
- Develop individual treatment plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Assist clients to access support services and establish community supports, and where needed are assisted through advocacy with service providers
- Monitor and review individual treatment plans and where needed action is taken to ensure the plan continues to meet client needs or goals
- Build strong and effective relationships with clients and their families, to assist client through treatment using family inclusive interventions
- Assist clients to build networks and post treatment support plans that include personal networks, after care, self-help groups and community activities- Work with Barwon Health to assess clients as suitable for the MH transitional housing program.
- Support participants in their move from Barwon Health’s acute MH Unit to transitional accommodation and participation in organised program activities
- Case manage participants exiting Barwon Health’s acute MH Unit including the development & review of individual case plans

You will have- Relevant tertiary qualification in Social Work or related discipline- Relevant experience in AOD and/or Mental Health- Experience in delivery of AOD/Mental Health client brief interventions- Experience and resilience in working with complex AOD/Mental Health clients presenting with challenging behaviours- AOD Skillset or willingness to undertake within the first 12 months of commencing employment- A Victoria Working with Children check- A current driver’s license
- A willingness to undergo a National Police Check
