About You
Are you are passionate about helping individuals living with mental illness and motivated to improve the lives of those experiencing crisis? Are you empathetic and gentle in your communication? Are you reliable and consistent in your practice, as well as being able to demonstrate the following attributes?

Patient and emotionally resilient with effective listening skills & an adaptable communication style
Capability to advocate for others and build sustainable support networks
Effective time management and responsiveness to prioritise and action critical incidents
Accurate in your reporting with fastidious attention to detail
Previous experience in mental health or case management (community or clinical setting), or qualification/s in mental health, social science or other relevant field

It is a requirement for our frontline staff to be fully vaccinated against COVID-19 and meet all compliance requirements in relation to NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s licence.

About the Role

Hospital Outreach Post-suicidal Engagement (HOPE) delivers psychosocial support to individuals who are at-risk of suicide, or have recently made an attempt to take their own life. HOPE is facilitated across a series of domains including community and social services, housing, finance, family support, cultural and spiritual support, education and employment as well as advocacy for all of the above. The provision of psychosocial support to eligible participants of HOPE is delivered as assertive outreach. Assertive outreach is a proactive approach to delivering support to non-engaged or at-risk clients where support is offered in different settings at times that are mutually suitable. Effective assertive outreach is characterised by sustained and persistent efforts to reach clients of HOPE at any suitable location and to work with them in partnership to improve their safety and quality of life.

**The role entails**:
Develop professional relationships with at-risk individuals, their families and other stakeholders, including consulting with allied health services and clinical staff
Assist clients with psychosocial tasks, and support them in developing goals and safety plans while identifying risk and employing crisis management practices
Make referrals to external services for continuing support for clients
An emphasis on high-quality and accurate reporting of client wellbeing, participation and plans to be presented in clinical reviews
Work in a multidisciplinary and risk-adverse environment, understanding the importance of process, clear and accurate communication in a challenging environment
