**SIL COORDINATOR POSITION DESCRIPTION**

**POSITION PURPOSE**

To provide leadership, manage and develop SIL staff, SIL staff supervision, direction for SIL staff on a day-to-day basis and oversight of the SIL property operations, whilst adhering to Best of Homecare’s policies and procedures and demonstrating our values.

**PERFORMANCE OBJECTIVES**

Your main responsibility as a SIL Coordinator is to provide on and off-site support to SIL staff and participants residing in the SIL homes with their day-to-day living, ensuring that they have maximum opportunity for developing quality lifestyles. You will provide direction to SIL staff and participants with the purpose of building quality relationships, which are supportive and life-enhancing. You will provide opportunities for choice, self-advocacy and participation in matters relating to the participant’s home as well as facilitating access to community services and facilities. In addition to adhering to Best of Homecare’s policies and procedures, you are responsible for:

- Coordinating and overseeing the delivery of SIL services.
- Developing and implementing individual planning documents
- Providing guidance and support to SIL staff
- Monitoring the quality of services provided
- Ensuring compliance with relevant policies and regulations
- Conducting work without direct supervision, but within a directed framework.
- **Detailed Responsibilities of the Role**:_
- Investigating any WHS incidents, and ensuring the WHS Incident and Hazard Report form has been completed correctly.
- Investigating and closing level 2 incidents and working with the compliance team in the investigation of level one incidents.
- Ensuring safe working conditions and implementing safe working practices in all work areas.
- Reviewing the Maintenance Register and SIL Safety Checklists prior to attending sites to check for newly identified hazards.
- Develop individual planning documents for new clients within 2 weeks of signing the service agreement.
- Ensure participants are supported to select their preferred workers, including gender, and to work with the rostering team to ensure that outlined preferences are respected.
- Monitor the participants’ progress and adjust planning documents as needed to ensure that they are receiving support that meets their needs, goals and preferences including updating documents within mandatory review periods
- Complete Roster of Care Tool for BHC participants (where required)
- Participate in Care Team meetings where required
- Monitor progress notes for quality and contents and provide feedback to SIL staff
- Review and collate behaviour reporting charts and report findings in weekly SIL Coordinator Meetings and to the Behaviour Support practitioner.
- Monitor Behaviour Support Plan implementation to ensure staff are implementing as per the practitioner’s guidance.
- Attend and contribute to team meetings and education sessions.
- When required, be involved in on-the-job training and/or orientation of new SIL staff.
- Coordinating SIL staff to take participants to appointments.
- With the care team, build relationships with families and significant others which promote inclusion, participation and partnerships in care.
- Providing leadership and identifying training needs of SIL staff.
- Conduct weekly 3-hour site visits to allocated SIL Homes and complete relevant reporting requirements.
- Effectively managing the performance by completing supervison call for the SIL staff to ensure that work is performed to the required standards.
- Communicate effectively and appropriately with participants, their family, care team, friends and associated professional workers.
- Action any minor complaints with participants and escalate complaints to the Compliance Support Officer
- Ensuring relevant and accurate information is recorded and maintained.
- Ensuring security, privacy, and confidentiality of all records.
- Actioning any identified risks to/from the participant and notifying the Director (Arun) if needed
- Conducting incident investigation for all level one
- Assisting in the investigation of complaints and incidents, resolving issues for participants or their supporters.
- Report staffing and conduct concerns to Human Resource Department
- Reporting weekly using the reporting template to Director (Arun)
- **Screening Requirements**:_
- 100 points of ID.
- Right to Work in Australia.
- NDIS Worker Screening Check.
- An international police check (where applicable).
- Working with Children Check.
- **Key qualifications, skills and competencies**:_
- NDIS Worker Orientation Module Certificate.
- Infection Control Training Certificate.
- Relevant degree in disability, mental health or aged care (desired).
- Demonstrated experience in team leadership.
- Thorough understanding of Supported Independent Living and best practice approaches.

Pay: $38.00 - $40.00 per hour
