**About the role**
cohealth's Older Persons Social Support and Respite programs offer Social Support Groups, centre-based Respite, and Flexible Respite programs to older people with low to high care needs and their carers.
The Lifestyle Support Worker provides day-to-day operational support for program planning and delivery, providing meaningful group and individual social activities designed to optimise consumers' health and well-being, build capacity, and prolong independent community living.
The Lifestyle Support Worker will work from Monday to Thursday in multiple locations including Niddrie, Flemington and Carlton.

**Your duties include**:
- Plan and facilitate group activities to older people
- Participate in client transport, including driving the centre vehicles, inclusive of an 8-seater van
- Ability to use technology for information gathering, client management and communication
- Establish respectful professional relationships that have clear boundaries with clients, staff and partner organisations

- Certificate 3 in Aged Care or equivalent and/or Certificate 4 in Leisure and Health
- Demonstrated knowledge and skills in the planning, implementation and evaluation of Social Support and Flexible Respite programs that meet the needs of older people, people with disabilities, and people with memory impairment
- Victorian Driver's License
- Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination._
