**The Opportunity**

Our client is seeking experienced Clinical Case Managers to join their team based in Sydney and Tamworth.

Key responsibilities include:

- Conduct comprehensive assessments of patient needs and develop individualized care plans in collaboration with the interdisciplinary team
- Coordinate services and resources necessary to support patients in achieving their goals
- Monitor patient progress and adjust care plans as needed to ensure desired outcomes
- Provide ongoing support and education to patients and families regarding disease management, coping skills, and community resources
- Collaborate with physicians, nurses, social workers, and other healthcare providers to ensure continuity of care
- Complete all necessary documentation and maintain accurate records in accordance with regulatory requirements and departmental policies
- Participate in quality improvement initiatives to enhance the delivery of patient care
- Attend regular team meetings and participate in case conferences to ensure coordination of care and the identification of complex cases

To be successful in the role, you will have:

- Current AHPRA registration
- Bachelor's or Master's degree in nursing
- Strong assessment, care planning, and case management skills
- Knowledge of community resources and healthcare delivery systems
- Excellent communication and interpersonal skills
- Ability to work independently and as a member of an interdisciplinary team
- Strong organizational and time management skills
- Experience with electronic medical records and documentation
- Ability to work in a fast-paced environment
