- About us:
Your Way Support Services is a Disability Service Provider offering In-Home support, and Community Access to people with Disabilities throughout the Moreton Bay, Sunshine Coast, Gympie Region, and the Gold Coast.
Your Way is a certified provider under the NDIS (National Disability Insurance Scheme) and pride ourselves on working closely with our Members (participants) and Support Workers to achieve their individual goals.
We are a young fast-growing company that is seeking a hands-on Case Manager to work within our Member Services Team.
About the Role:
**Responsibilities**:
- Preparing and submitting budgets for service delivery
- Preparing and submitting service agreements
- Assist with recruitment for your members
- Assist with scheduling of shifts for your members
- Monitoring and responding to NDIS plan changes.
- Ensuring administration procedures are adhered to.
- Keeping accurate details on members through the company’s CRM.
- Ensuring compliance under the NDIS Commission.
- Liaising with other departments on matters pertaining to members.
- Ensuring all files are always up to date and compliant.
- Meeting with potential new members.
- Promotion of services (Business Development) to potential new members and their families.
- Building relationships/networking with potential members, referral sources, etc.
- Maintaining relationships with your members, families, and stakeholders.
Skills and Qualifications:
- Intermediate/high experience in Microsoft Programs.
- Qualifications within community/support.
- Minimum 3 years of demonstrated experience in a similar role or Case Management/Care coordination.
- Capacity building and skills development with people with disabilities.
**Requirements**:
- Current First Aid and CPR Certification.
- Current NDIS Workers Screening and Blue Card.
- Current valid Driver’s Licence.
- Excellent Networking Skills.
- Excellent Time Management and Organisational Skills.
- Capacity to engage constructively and respectfully with individuals of diverse backgrounds and abilities to achieve high-quality outcomes.
- Ability to work autonomously, under limited supervision, and have the ability to take the initiative, when required.
Highly Desirable:
- Strong written and verbal communication skills.
- Excellent administrative skills.
- Professional phone manner and excellent customer service.
Successful applicants will be required to undergo a current National Police Check.
WAGES reviewed upon successful completion of 6 month probationary period.
**Salary**: $55,000.00 - $65,000.00 per year
Schedule:
- 8 hour shift
