We are currently looking for a Full Time **NDIS Service Coordinator** to join a small team focused on quality outcomes. You will assist and support NDIS participants, their families and carers to enable them to implement their goals and aspirations.

Working as part of a disability support team the **NDIS Service Coordinator** will maintain an effective and consistent approach to service delivery under the general direction of the NDIS Service Manager. The position requires the person to have a commitment to the aims and objectives of **Mana Wellness Disability Services**.

**Flexible working and a 9 day fortnight (post probation) is available.**

**Service Coordinator Responsibilities and Duties.**

The role includes, but is not limited to:

- Managing the day-to-day operation of the SIL houses
- Liaising with Support Coordinators, participants, their families, allied health professionals, and other relevant stakeholders to ensure a high standard of service delivery.
- Building strong relationships with all stakeholders
- Managing the client intake process
- Writing support plans and undertaking risk assessments for participants **Leading, mentoring, and developing staff.**:

- Rostering and other administrative tasks
- Facilitating staff team meetings
- Encouraging and supporting weekly meetings for the SIL houses including menu and activity planning
- Medication Management and Behaviour Support supervision and monitoring
- Reporting restrictive practices to relevant authorities
- Preparing quotations
- Preparing Service Delivery Reports 3 monthly
- Ensuring company is delivering supports sustainably.
- Site Visits of SIL and Community Clients to ensure safety and quality of supports being delivered

**About you**:

- You are an experienced support worker. You currently hold a minimum of a Cert 3 in Individual Support, Disability or a related discipline or be working towards higher qualifications in Disability or Community Services.
- Previous experience in Service Coordination in disability services.
- A Good understanding of SIL, STA, MIL, Restrictive Practice prevention
- You are adaptable, think on your feet and can problem solve effectively.
- You’re a proactive support worker, who meets all the requirements of that role.

**Job Type**: Part-time

Pay: $75,000.00 - $88,000.00 per year

Expected hours: 34 - 38 per week

Schedule:

- Day shift
- Fixed shift
- Night shift
- Weekend availability

Work Authorisation:

- Australia (preferred)

Work Location: In person
