We are seeking Support Coordinators who have experience in interpreting and coordinating NDIS participant plans, assisting them to establish their identified supports to include formal, mainstream, and funded supports maximising independence, choice and decision making for participants.

**What you bring**:

- Minimum Certificate IV in Disability or equivalent with substantial knowledge and experience within the sector
- Full current Victorian Driver license (or equivalent issued by Australian state or territory)
- NDIS Worker Screening check
- Comprehensive knowledge of person-centred principles and approaches, including positive behaviour support
- Strong computer skills, with experience in MS Office and client data management systems
- Demonstrated ability to adapt communication style to build and maintain diverse connections and relationships with service providers, individuals and/or families/carers
- Demonstrated ability and willingness to work in a competitive market environment where required to meet targets
- Capacity to implement change and identify opportunities for innovation

**Desirable**:

- Knowledge and experience of the NDIS Support Coordination purpose and processes
- Knowledge of relevant service providers across relevant community service sectors
