**About the departments**:
The Department of Clinical Haematology sits within Austin Health’s Division of Medical and Cancer Services, and forms part of Austin Health’s Olivia Newton John Cancer Wellness and Research Centre. Clinical Haematology provides services for a wide range of haematological conditions in the inpatient, specialist clinic, same-day treatment and home-based care settings. Apheresis and cellular therapies (including autologous and allogeneic stem cell transplantation) form part of the service. A 24-hour consultative service is also provided throughout Austin Health. Close collaboration occurs with Austin Pathology, particularly with the Department of Laboratory Haematology.
Hospital in the Home and Virtual Care (HITH-VC) is a multi-disciplinary team providing seven day/week home-based services and aligns with a key direction of providing more care outside the hospital. Within HITH-VC a new model of care has been developed to support higher acuity patients utilising virtual care.
Virtual Care operates to care for patients with cardiac conditions including heart failure, atrial fibrillation and post cardiac surgery care, and haematology conditions, including leukaemia and autologous stem cell transplants, caring for them in their home environment, in a multidisciplinary team setting.
**About the role**
- Hours per week: 10.5 hours per week in total (shared between 2 clinicians i.e. 5.25 hours each).
- Employment status: Permanent
- We are looking to appoint two clinicians who will each work alternate weeks (ie: 10.5 hours per fortnight, paid 5.25 hours per week).
As the Hospital in the Home and Virtual Care (HITH-VC) Haematologist, you will be responsible for the following clinical responsibilities on a 1:2 week roster:
- Provide comprehensive management of allocated patients which may include haematologic malignancies, including but not limited to acute leukaemias and lymphoproliferative disorders; disorders of haemostasis and thrombosis and other haematologic conditions.
- Manage Clinical Haematology patients receiving treatment on specific HITH-VC Haematology care pathways, in conjunction with Haematology Clinical Nurse Consultants (CNCs) and HITH-VC nurses.
- Primary contact for caring for Clinical Haematology patients managed on general HITH-VC pathways during work hours (0800-1700 Monday to Friday excluding public holidays).
- Provide haematologic consult advice during work hours for non-haematology patients managed by HITH-VC (0800-1700 Monday to Friday excluding public holidays).
**About you**
To be successful in this role you will:
- Be a registered Medical Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
- Hold a Fellowship of the Fellowship of the Royal Australasian College of Physicians (FRACP) or equivalent
- Have appropriate training and experience applicable to the field of Haematology.
- Hold certification of FRACP continuing professional development (CPD);
- Hold AHPRA specialist registration in the field of haematology.
- Demonstrate a commitment to high quality patient care
