**About Hospital in the Home (HITH)**
Hospital in the Home shifts traditional acute care to the patient's own home, without compromising patient care. This may take place in person, or increasingly via the HITH virtual ward utilising video Telehealth and wearable technology. Care in Hospital in the Home is becoming increasingly acute thanks to the increase in access to technology. HITH improves patient satisfaction, reduces costs per episode of care, reduces complications and improves hospital flow.
**About the role**
The HITH Case Manager manages the transfer of patients from a traditional ward environment to receiving care in their own home. They manage the needs of patients collaboratively to ensure high patient satisfaction, while facilitating hospital flow and safe and effective transitional care.
They utilise their extensive clinical skills and experience to manage clinically complex patients, utilising resources (such as wearable technology) to ensure the patient receives the right care, and are able to escalate appropriately.
A regular day will see you:
- Reviewing referrals to the HITH service to ensure appropriate care can be put in place (be that through facilitation of in-person nursing care, virtual care, or a combination of the two).
- Assessing patients, including assessment of ECG and other clinical information
- Supporting flow in Austin Health by finding novel solutions to complex patient needs
**About you**
To be successful in this role you'll bring with you:
- A very high level of clinical skill and experience, allowing for high level reasoning and complex decision making
- Preferably a Cardiology or Critical care post grad qualification - to allow for appropriate management of Complex patients, particularly in the virtual space.
- Strong level of comfort utilising online systems for clinical management, and confidence in learning new systems
- A desire to work collaboratively in a growing area, with a positive mindset towards change and innovation.

Selection Criteria Essential: Registered Nurse registered with the Nursing and Midwifery Board of Australia
Essential: Demonstrates comprehensive skills in acute nursing.
Essential: High level of competency in solving IT issues, and a willingness to learn new technologies.
Essential: A sound understanding of information technology including clinical systems, as required for the department.
Essential: Current Victorian Driver’s license.
Essential: Excellent interpersonal and organizational skills
Essential: Ability to negotiate and gain co-operation and support of others.
Essential: Ability to determine workload priorities and meet deadlines.
Essential: A positive approach to ongoing self-education and skill development
Desirable: Post Graduate Certification in Cardiac/Critical Care Care Nursing.
