**Salary: $47,205.95 - $51,792.45 (pro rata) per annum**

**Position Summary**:
The Transitional Aged Care Program (TACP) provides a package of services for up to 12 weeks who after admission to hospital, meet a set criteria for a home-based aged care package and who may have the potential to manage at home with rehabilitation and support services.

This exciting position involves the skilled Assistant in Nursing (AIN) interested in working to work in an aged care community setting to provide care to people living at home in the Eastern Suburbs and inner Sydney area home who are referred to the Transitional Aged Care Program.
The Assistant in Nursing will work within a multidisciplinary team under the direction and supervision of experienced clinicians to provide assistance with personal care, light domestic duties, and providing health monitoring and therapeutic support to clients of the program to maximise improvement in their functional abilities.

The vacancy is open to Australian citizens, New Zealand citizens who hold a valid New Zealand passport, Australian permanent residents and those non-residents deemed by the Department of Immigration and Citizenship to be eligible to work in Australia only.

**Selection Criteria**
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia.

2. HLT32507 Certificate III in Health Services Assistance (Assisting in Nursing Work in Acute Care) or equivalent.

3. Current unrestricted NSW drivers licence - Class C or above

4. Demonstrated effective interpersonal, written and verbal communication skills with the ability to communicate with a range of people

5. Demonstrated ability to prioritise workload, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required) as delegated by supervising Health Professional

7. Demonstrated experience working with older people in a community/rehabilitation setting
