We are seeking a passionate and reliable **Disability Support Worker** to join our individualised support team. This role supports a **young female participant** in a private residence, focusing on independence, inclusion, and personal goals under the NDIS.

You will work closely with the **House Coordinator / Team Leader** and other support staff to provide tailored, person-centred care in a private residence.

**What You’ll Do**:

- Provide respectful, high-quality personal care and mobility support
- Assist with daily living - meals, cleaning, laundry, shopping
- Support community outings and engagement in activities
- Follow and contribute to the participant’s care plan and goals
- Advocate for the participant’s rights and independence
- Ensure safety, well-being, and positive risk management
- Maintain accurate documentation in line with NDIS standards
- Collaborate with team members and allied health professionals

**About You**:
You are empathetic, motivated, and driven to make a difference in one person’s life. You bring a proactive mindset, attention to detail, and a strong team ethic.

**Essential Requirements**:

- Experience in disability or supported independent living (SIL)
- Strong understanding of person-centred and active support
- Excellent communication and interpersonal skills
- Proven reliability and initiative
- **NDIS Worker Screening Check (mandatory)**
- **First Aid & CPR (required or willingness to obtain)**
- **Epilepsy & Medication Administration training (required or willingness to obtain)**
- **Driver’s licence and fully insured, reliable vehicle**

**Desirable**:

- Certificate III or IV in Disability, Individual Support or related
- Experience working with individuals with complex needs
- Understanding of NDIS plans and participant-focused supports

Pay: From $35.00 per hour
