About the GPIU

The General Practice Integration Unit (GPIU) is a joint collaboration between Adelaide PHN (APHN), and the Central Adelaide Local Health Network (CALHN) and is located in the Central Adelaide metropolitan region.

The GPIU is tasked with improving the continuity of care of people across settings by facilitating and strengthening collaboration, communication and integration between general practice and the hospital.

About the Role

**The GPIO role will involve**:
Fostering and enabling engagement and relationship building between CALHN clinicians and general practice in the Central Adelaide region to promote seamless patient transition across the health care system.

Working with General Practitioners to improve the quality of clinical handover from primary to acute care in the CALHN region.

Working with the CALHN services to improve the quality and timeliness of clinical handover on discharge back to the person’s usual General Practitioner.

Timely actioning of escalated/complex queries from general practices relating to unclear or missing CALHN correspondence, clinical plans for patients and waitlist management.

Participating in relevant meetings, workshops and system design processes that have significant general practice and hospital interface requirements and align to the GPIU workplan.

Contributing to newsletters and other forms of communication.

This work will be situated in the GPIU and will be based across the Central metropolitan region (initially within the RAH and potentially the Sefton Park location). The GPIU team also includes a service portfolio lead, another GPIO (0.2FTE) and a clinical nurse.
