**Position Title: Aboriginal Cancer Care Co-Ordinator**

**Location: Gosford Hospital**

**About The Role**

The Aboriginal Cancer Care Coordinator (ACCC) will support and participate in health promotion and educational activities that promote the benefits of screening and early intervention and preventative measures. ACCC to build trust and positive working relationships with Aboriginal and/or Torres Strait Islander patients, their families, and the broader community to promote cancer services through community education initiatives. ACCC to provide guidance and support to ensure Aboriginal and/or Torres Strait Islander patients and their families are informed and prepared for cancer treatment. ACCC to promote accessibility and encourage engagement with cancer services, with the objective to remove barriers to accessing and completing cancer treatment.

**Key Accountabilities**
- **Stakeholder Collaboration**: Work closely with key Aboriginal stakeholders, including non-government organizations, cancer services, and local health district Aboriginal health units. Ensure their active participation and valuable contributions to program development, leading to successful project delivery.
- **Cultural Sensitivity and Community Relations**: Foster strong links and relationships with Aboriginal communities and agencies, promoting a culturally acceptable and competent program delivery. Provide sensitive and holistic care aligned with cultural principles for both Aboriginal communities and staff.
- **Holistic Care Coordination**: Collaborate with a diverse team to assess, plan, organize, evaluate, and deliver culturally safe and secure holistic care. Ensure Aboriginal patients and their families are well-prepared for cancer treatment, navigating systems for timely and consistent care.
- **Access and Engagement Advocacy**: Concentrate on enhancing accessibility and engagement with cancer services by actively addressing barriers. Support and engage with key Aboriginal community groups to educate and advocate for screening, prevention, and address social and emotional wellbeing issues.
- **Project Planning and Reporting**: Develop and implement a comprehensive project work plan for the Coordination of Care Project. Ensure a progressive approach over a 12-month period to achieve the Pilot project's goals. Contribute to ongoing program documentation, including status updates, reports, discussions, and achievements, to keep stakeholders well-informed.
- **Administrative Proficiency**: Handle administrative responsibilities, including record maintenance, data collection, and documentation, with a focus on maintaining client confidentiality. Demonstrate proficient computer skills using various software programs for effective word processing, data management, and spreadsheet utilization.

**About You**
- This is an Identified Aboriginal/Torres Strait Islander position. Applicants for this position must be of Aboriginal descent through parentage, identification as being Aboriginal and being accepted in the community as such. - Active and demonstrated involvement in the local Aboriginal community, with demonstrated understanding and commitment to improving the health issues impacting on Aboriginal communities, including a knowledge of relevant trends, issues, policies, and practices.
- Completed or enrolled in relevant tertiary qualifications in health-related discipline and/or relevant experience.
- Demonstrate capacity to engage key partners/participants within the Health Service and external groups/organisations and anticipate their needs.
