**Your Story Community Supports** is seeking compassionate, dedicated, and professional **Support Workers** to join our team at our **Shell Cove Group Home**.
This rewarding opportunity involves providing high-quality support to residents with a range of needs, including **mental health support, personal care, hoisting, mobility assistance, and community participation**.

**About the Role**:
As a Support Worker, you will play a vital role in assisting residents to live independently, maintain their dignity, and achieve their personal goals. You will work within a structured environment, supporting individuals with varying levels of physical and mental health needs.

**Key Responsibilities**:

- Provide respectful, person-centred personal care (e.g., showering, grooming, toileting).
- Assist with manual handling, including hoisting and mobility support.
- Implement individual support plans and behaviour support strategies.
- Support residents with mental health conditions to maintain stability and independence.
- Administer medication (with appropriate training and competency assessment).
- Support residents with daily living tasks including cleaning, meal preparation, and shopping.
- Promote community access, social inclusion, and participation in meaningful activities.
- Accurately document client progress, incidents, and daily notes.
- Maintain a safe, clean, and supportive environment in line with workplace health and safety standards.
- Work collaboratively with other team members, families, guardians, and health professionals.

**Essential Criteria**:

- Previous experience supporting individuals with mental health needs and/or complex physical support needs.
- Competency and experience in safe hoisting and manual handling procedures.
- Current First Aid and CPR Certificate.
- Current NDIS Worker Screening Check (or willingness to obtain).
- Valid Australian Driver’s Licence and reliable, comprehensively insured vehicle.
- Proof of Australian work rights.
- Ability to work flexible shifts, including mornings, evenings, weekends, and public holidays.

- Rotating roster
- Shift work
- Weekend availability
- Weekends only

Supplementary Pay:

- Performance bonus
