Your Story Community Supports is seeking compassionate, dedicated, and professional Support Workers to join our team at our Shell Cove Group Home.

This rewarding opportunity involves providing high-quality support to residents with a range of needs, including mental health support, personal care, hoisting, mobility assistance, and community participation.

About the Role:

As a Support Worker, you will play a vital role in assisting residents to live independently, maintain their dignity, and achieve their personal goals. You will work within a structured environment, supporting individuals with varying levels of physical and mental health needs.

Key Responsibilities:

  • Provide respectful, person-centred personal care (e.g., showering, grooming, toileting).
  • Assist with manual handling, including hoisting and mobility support.
  • Implement individual support plans and behaviour support strategies.
  • Support residents with mental health conditions to maintain stability and independence.
  • Administer medication (with appropriate training and competency assessment).
  • Support residents with daily living tasks including cleaning, meal preparation, and shopping.
  • Promote community access, social inclusion, and participation in meaningful activities.
  • Accurately document client progress, incidents, and daily notes.
  • Maintain a safe, clean, and supportive environment in line with workplace health and safety standards.
  • Work collaboratively with other team members, families, guardians, and health professionals.

Essential Criteria:

  • Previous experience supporting individuals with mental health needs and/or complex physical support needs.
  • Competency and experience in safe hoisting and manual handling procedures.
  • Current First Aid and CPR Certificate.
  • Current NDIS Worker Screening Check (or willingness to obtain).
  • Valid Australian Driver's Licence and reliable, comprehensively insured vehicle.
  • Proof of Australian work rights.
  • Ability to work flexible shifts, including mornings, evenings, weekends, and public holidays.
