**Where you’ll be working**

You will be based at the Walker Building, Wallsend Campus, as part of the Aboriginal Health Care Coordination (AHCC) service. This role sits within an Aboriginal team, delivering care through an Aboriginal-led model of care across the district.

This area of health is dynamic and continually adapting to ensure the needs and voices of Aboriginal communities remain at the centre of care. You will be supported by a strong Aboriginal Health team and collaborate closely with hospitals, community providers, and Aboriginal Community Controlled Organisations.

**What you’ll be doing**

As an Aboriginal Health Practitioner, you will play a central role in delivering flexible, holistic, and culturally safe healthcare to Aboriginal and Torres Strait Islander peoples, their families, and communities. Working within a multidisciplinary team, you will provide direct clinical care within your Scope of Practice, advocate for patients and families, and support culturally responsive care planning.

The AHCC service provides up to 28 days of coordinated care post-discharge to empower Aboriginal patients, strengthen follow-up, and improve long-term health outcomes. Aboriginal Health Practitioners and Workers are at the heart of the program, acting as cultural connectors and clinical supporters to ensure care is delivered in a safe and respectful way.

This position will:

- Deliver direct clinical services as defined by your Scope of Practice.
- Support patient care navigation, follow-up, and continuity across hospital, community, and primary care settings
- Provide culturally appropriate health education and yarning-based support.
- Advocate for safe and equitable access to services for Aboriginal people.
- Collaborate with Aboriginal Health Workers, clinicians, and community organisations to ensure seamless, culturally safe care. Maintain accurate documentation in line with NSW Health standards.

- Confidence in working both independently and as part of a multidisciplinary team.
- Knowledge of Aboriginal health services and the barriers Aboriginal people may face when accessing care.
- A passion for improving Aboriginal health outcomes and patient experiences.
- Strong communication, relationship-building, and advocacy skills.

**Requirements**:

- Certificate IV in Aboriginal Primary Health Care (Practice) and current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Aboriginal Health Practitioner

**Additional Information**:

- To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
