POSITION Support Worker in Community Engagement

REPORTS TO Team Leader

**position description**

Purpose of role
- To support the Team Leader and participants in facilitating the programs and activities to enhance the physical, emotional and social well-being and independence of people with disabilities based on their individual needs and interests

Main duties and responsibilities
- supporting your manager to run your workplace according to standards and legislation
- acting as a positive role model to colleagues
- facilitate programs that outline what support individuals need
- working in partnership with other professionals to ensure individuals get the support they need.
- Help create a monthly planner for group activities or individual activities - Catering to participants choice and interests.
- Promoting the dignity, independence and rights of each participants
- Encouraging Participants to develop personal, community and social relationships and leisure, sport, recreation holiday and travel skills as appropriate
- Providing appropriate support to participants to enable them to achieve their individual goals.
- Assist participant’s with adl’s including but not limited to personal care, medication management, travel support.
- The above lists are not exhaustive, and the role may change to meet the overall objectives of the company.

**Qualifications**:

- _Cert 111/1V aged/disabilities / Leisure and health and or working towards_

Experience
- _Experience in community settings_
- Knowledge
- Work tasks are planned and clearly defined, with established procedures well understood.
- Support worker may only alter the planned task if there is a risk to participants safety and inform Team Leader
- Support worker may resolve minor problems that relate to immediate work tasks using procedures or practices alongside the Team Leader
- Actively promote and facilitate participants involvement in the delivery and evaluation of the support provision.

- **Communication**:the ability to communicate clearly and concisely**, **varying communication style depending upon the audience.
- Attention to detail: excellent attention to detail and written skills when communicating with others, both internally and externally.

- Time management/organisation: accomplish objectives effectively within time frame given and carry out administrative duties within portfolio in an efficient and timely manner.
