Support Relationship Manager

We are looking for a **Support Relationship Manager** to join our dedicated team at Support Umbrella Group. In this rewarding role, you will be the key contact for NDIS participants, helping them navigate their support plans and ensuring they receive high-quality, person-centred care.
**About the Role**:
As a Support Relationship Manager, you will play a crucial part in maintaining and growing relationships with our participants, their families, and other service providers. Your main responsibility will be to ensure that participants receive the support they need, when they need it, by coordinating services, managing care plans, and advocating for their rights.
**What You’ll Do**:
- Build and maintain strong relationships with participants, carers, and stakeholders.
- Manage participants’ NDIS plans, coordinating the delivery of support services.
- Ensure compliance with NDIS guidelines and maintain accurate records.
- Address participant feedback and resolve service-related issues promptly.
- Work closely with support workers and team leaders to ensure seamless care delivery.
**What We’re Looking For**:
- Minimum 2 years of experience in a similar role within the NDIS or disability sector.
- Strong understanding of the NDIS framework and plan management.
- Excellent communication and interpersonal skills.
- Proven ability to coordinate services and manage multiple participants.
- A passion for delivering high-quality, person-centered support to individuals with disabilities.

**Job Types**: Part-time, Casual
Pay: $25.71 - $64.14 per hour
Expected hours: 10 - 30 per week
