**About the role**

An exciting opportunity is available to be a part of our care communities new Meaningful Life Model of Care, with opportunities across Ballarat and Horsham.

The coordinator is responsible for the development, implementation and coordination of meaningful engagement that gives daily purpose to our residents, bringing joy to all that we care for. This will be delivered through meaningful activities and purposeful engagement that supports a holistic wellbeing.

**Key Responsibilities**
- Develop, implement and coordinate purposeful engagement across the care community, which meets individual residents’ goals, needs and preferences while promoting choice.
- Provide leadership, mentor and support the meaningful life team, volunteers and students in the care community.
- Build connections with the community to deliver high quality innovative and person-centred engagement.
- Coordinate and be responsible for all meaningful life assessments, care plans, residents’ profiles and documentation.
- Attend and provide reports / feedback at resident, relative and care community team meeting. Coordinate regular resident and relative surveys/ audits, and report progress and outcomes.
- Coordinate rostering, training and professional development for direct reports.
- Respond in a timely manner to any complaints or concerns raised.

**Skills and experience**
- Hold a Certificate III in Aged Care (or equivalent) or Certificate IV in Leisure and Health (or equivalent).
- A Qualification in dementia care is desirable.
- Demonstrate an understanding of Aged Care Quality Standards.
- Excellent communication and organisational skills, with strong computer and customer service skills
- Experience with the Aged Care software program iCare is desirable.
- Victorian Driver’s Licence
